Inferior shoulder dislocation with greater tuberosity avulsion fracture treated with Traction-Countertraction method in a rural hospital: A case report and literature review

Introduction. Inferior shoulder dislocation is considered a rare event. Although there seemingly low chance of occurrence, this condition should not be prematurely ruled out in the presence acute pain. Case. A 45 years old woman complained pain her left and was unable to bring arm lower than 90° after car accident. The patient unintentionally put outstretched above head avoid impact during crash, causing hyperabduction shoulder. She experienced pain, numbness could move ipsilateral hand. Her locked (100°), elbow extended. Close inspection showed loss contour. On palpation, humeral palpable axilla. Anteroposterior X-ray view confirmed inferior with associated greater tuberosity fracture. closed reduction under general anesthesia traction-counter traction technique conducted as commonly used rural settings. After perfect anatomical obtained X-ray, immobilized using an sling. for three weeks adducted position support soft tissue healing. Physiotherapy started weeks. Conclusion. orthopedic pathology that easily dismissed post-traumatic mechanism injury characterized by downward force or lever on hyperabducted arm. Orthopedic surgeons areas utilize Traction-countertraction method.
